Croydon Children’s Continuing Care: Support for Families with Complex Health Needs

Croydon Children’s Continuing Care is a service that provides extra help for children and young people with serious or complex health needs. These are children who need ongoing care and support beyond what is usually provided by local health or social care services.

This support is for children and young people aged 0-18 who may have needs due to:

  • Long-term health conditions
  • Complex disabilities
  • Life-limiting or life-threatening conditions
  • Medical needs that require care at home, school or in the community 

What does Continuing Care mean?

Continuing Care means your child may need:

  • Help from nurses, carers or trained support workers.
  • Specialist equipment or care routines.
  • Support at home, in school, or during short breaks.
  • Care overnight or throughout the day.
  • A personalised care plan based on their health needs.

This support is arranged by NHS South West London ICB (Integrated Care Board), working closely with local services in Croydon. 

Who can get Continuing Care?

To receive Continuing Care, your child’s needs must meet the national Children’s Continuing Care Framework. This means they need a high level of care due to their health condition.

The first step is a referral. This can be made by:

  • A health professional (like your child’s nurse, doctor or paediatrician).
  • A social worker or member of your child’s education team.

Once a referral is made, a health assessment will take place.

The Continuing Care team will work with you to understand your child’s needs and decide if extra support is needed. 

What support is available in Croydon?

If your child qualifies for Continuing Care, you may receive:

  • A personalised package of care delivered at home or in the community.
  • Help with training to carry out medical tasks.
  • Short breaks or respite to give families time to rest.
  • Support in early years settings or schools, where needed.
  • A named health professional who will support your family.

The service works alongside schools, children’s community nursing, social care, and other services involved in your child’s care.
