The Community Children's Nursing Team is an integrated team of Community Children’s Nurses, nurse specialists, special school nurses, Drama Therapists, nursery nurses, healthcare assistants and administrative support that provides a service for children at home, and within Croydon Special Schools, with additional health needs.

The service provides support across Croydon for babies, children and young people either at home, in school, clinic or ward setting, including:

• Acute nursing care to reduce the need for hospital admissions and support early discharge when they do have to go to hospital.
• Care co-ordination for children with long term and complex conditions.
• Teaching and education packages for children and families so that they can independently manage their health needs.
• Palliative and end of life care for babies, children and young people with life threatening and life limiting conditions.
• Supportive play and distraction for children and young people with nursing needs.
  
• Support for children, young people and siblings through ‘Willow’ children’s dramatherapy bereavement service.
• Nurse Specialist service for children and young people with diabetes.  
  
• Nurse Specialist single intervention service for children and young people with asthma.
• Oncology Nurse Specialist.
• Special School Nursing Service for children and young people aged 2-19 who have additional medical needs attending Croydon special school and nursery settings.
  
• Family Support. 

You can contact the Children’s Community Nursing Team Monday-Friday between the hours of 08.00-18.00.

They also offer a limited weekend service between the hours of 09.00-15.00 on Sunday and Bank Holidays.
