Community Children's Nursing Team

The Community Children's Nursing Team is an integrated team of children's nurses and Specialist Practitioner Community Children's Nurses, working closely with specialist nurses, the Tulip Nursing Team and Drama therapists. They provide support at home and to children and young people with additional health needs in mainstream school provisions. 

 The service provides support across Croydon for babies, children and young people either at home, in school, clinic or ward setting, including:

  • Acute nursing care to reduce the need for hospital admissions and support early discharge when they do have to go to hospital.
  • Care co-ordination for children with long term and complex conditions.
  • Teaching and education packages for children and families so that they can independently manage their health needs.
  • Palliative and end of life care for babies, children and young people with life threatening and life limiting conditions.
  • Supportive play and distraction for children and young people with nursing needs.
  • Support for children, young people and siblings through ‘Willow’ children’s dramatherapy bereavement service.
  • Nurse Specialist service for children and young people with diabetes.  
  • Nurse Specialist single intervention service for children and young people with asthma.
  • Oncology Nurse Specialist.
  • Training and Individual Health Care Planning for children and young people with additional health needs in mainstream school settings.
  • Non-medical prescribing with nurse led treatment plans.
  • Health representation for CYP receiving support through Child in Need or Safeguarding Plans
  • Family Support.

You can contact the Children’s Community Nursing Team Monday-Friday between the hours of 08.00-18.00.

They also offer a limited weekend service between the hours of 09.00-15.00 on Sunday and Bank Holidays.
